Adult Services Program

The Traumatic Brain Injury Trust Fund Services Program provides care coordination to help individuals with TBI and their families connect to service providers and community resources. The Trust Fund also provides a limited amount of funds for needed services related to rehabilitation from the traumatic brain injury.

Overview of Care Coordination

TBI Trust Fund contracts with Colorado Connections, a division of Denver Options, Inc.,  to provide care coordination for adults no longer participating in high school and who have experienced a traumatic brain injury. The Care Coordinator completes an intake assessment and develops a care plan with the individual for one year of service. The Care Coordinator works with the individual to provide information, identify resources, coordinate services, and develop advocacy skills related to their TBI.

Purchased Services

Individuals in the Adult Services Program have access to limited funds provided by the TBI Trust Fund that can be used for services related to the TBI, not covered by insurance or other funding sources. The services must be pre-approved and are paid directly by Colorado Connections.

Individuals in the Brain Injury Community have access to a dynamic assortment of educational programs, workshops, and informational materials all related to brain injury.   These programs are offered statewide, click here to see what’s happening now. 

Application Process

To be eligible for the Adult Services Program, one must be a lawfully present in Colorado, U.S. citizen, have medical documentation of a TBI, have ongoing impairments as a result of a TBI, and complete the application and appropriate documentation.

Applications are available at the Brain Injury Association of Colorado (BIAC) or by calling 303-355-9969, or 888-331-3311. The BIAC staff can assist applicants with completing the application process.
